How to make a referral to Adult Social Care

There are many ways to make a referral to Adult Social Care

If you feel you or someone else have care and support needs, you can make a referral to Adult Social Care to consider what support might be available to you. 

For yourself, or someone else

You can find out more about What is adult social care and who is eligible for support as well as what happens after you've made a referral. 

If you would like to make a referral to Adult Social Care for yourself or someone else, you can do this online:

Worcestershire Adults Portal.

Referrals submitted through the portal will be reviewed by advisors who will consider the information you provide and either get back to you directly or will pass your enquiry on to the appropriate Adult Social Care team to contact you. 

If you are unable to use the portal or wish to contact the team to talk through your concerns by telephone, please call us on 01905 768053, Monday to Thursday 8:30am to 5pm, Friday 8:30am to 4:30pm

For a discussion about support available to you or for an assessment of your needs as a Carer you can self-refer to Worcestershire Association of Carers online using the Worcestershire Adults Portal

How the portal works

The forms you complete go to Here2Help and Adult Social Care. You will be asked to register for a unique account which allows you to view submitted forms and receive feedback about your referrals.

You only need to register once and referrals will be kept for 30 days.
