Describe your needs in your own words (this is commonly referred to as self assessment) by answering a series of questions relating to your health and well being.
How does the self-assessment work?
A self-assessment can be completed by a friend or relative with the consent of the person being referred, or in instances whereby the person completing the self-assessment has Lasting Power of Attorney.
The self-assessment involves answering a series of questions about your situation and your needs, this information will then be used to signpost you to sources of information, advice and support that is personalised to you.
Once you have completed this self assessment, you can either decide not to have any further input from us, or, you can submit your information for the Adult Front Door Team to review. The team will either contact you, or direct this to Adult Social Care for a more formal Needs Assessment.
