How to make a referral

You can send us a self-assessment which will help us to understand your needs and give you access to local and national support. Or, you can send a referral through to us via the adult front door. Find out more below.

Describe your needs to access information and support

Describe your needs in your own words (this is commonly referred to as self assessment) by answering a series of questions relating to your health and well being. 

How does the self-assessment work?

A self-assessment can be completed by a friend or relative with the consent of the person being referred, or in instances whereby the person completing the self-assessment has Lasting Power of Attorney.

The self-assessment involves answering a series of questions about your situation and your needs, this information will then be used to signpost you to sources of information, advice and support that is personalised to you. 

Once you have completed this self assessment, you can either decide not to have any further input from us, or, you can submit your information for the Adult Front Door Team to review. The team will either contact you, or direct this to Adult Social Care for a more formal Needs Assessment.

Adult Front Door referral

By making an adult front door referral, we can support you with:

  • information, advice, guidance and signposting
  • direct referrals to community support and services
  • referral to the Targeted Adult Support Team
  • referral to Adult Social Care

What to expect from us

One of our advisors will call you once you have made contact, they will:

  • discuss what is working well for you and what you need support with
  • determine the most suitable information, advice and guidance for you
  • communicate with you through your preferred method of contact
  • summarise your conversation and provide details about the information discussed and any recommended services

If the advisor determines that additional support is needed, they may refer you to the Targeted Adult Support Team or directly to Adult Social Care with consent.

How our referral portal works

The forms you complete go to the Adult Front Door service and Adult Social Care. You will be asked to register for a unique account which allows you to view submitted forms and receive feedback about your referrals.

You only need to register once and referrals will be kept for 30 days.

However, if you are a member of the public and cannot complete the forms online, please book a telephone appointment. 

Please note: the telephone appointment line is not for professionals. All professionals must use the online forms.
