Change your Blue Badge details
If you change your name, address or contact details you need to let us know.
If you are changing your name, address or both and you also need to renew an expiring badge, please do not use this form, please complete a renewal application. The renewal will ask for any changes. Information on renewing is on our website.
Important information
You should let us know if any of your details have changed:
- your name
- address
- email address
- telephone number
Please note that changing a name and replacing a badge will require a replacement Blue Badge at a cost of £10.
