Resources for teachers to support pupils at transition

Useful documents and resources for teachers.

It has long been recognised that transition between Primary and Secondary school is a critical period, for children, which can positively and negatively impact future emotional wellbeing and mental health which impacts on attendance and educational outcomes.

Here you will find guidance, Relationships, Sex, and Health Education (RHSE) lesson plans, assembly plans, activities, videos, questionnaires for pupils and parents to support Transition leads, SENCOs, Year Group Heads and Support Staff.

  • Moving up! The transition to secondary school | Anna Freud  - the Anna Freud Foundation offers a Teacher Toolkit for schools which includes:
    • an animation
    • a Lesson Plan and various lesson activities
    • an Assembly Plan and PowerPoint slide show
    • a list of helpful resources
    • a copy of the animation script
    • a blank signposting poster for you to print and use in school
    • lesson plans, activities, assembly and video
  • Supporting school transitions | Resources | YoungMinds - resources focused on transition provided by the Mental Health Charity Young Minds and includes:

  • Information, Downloads and Resources | UCL Psychology and Language Sciences  - these resources arose from the School Transition and Adjustment Research Study (STARS) and includes:
    • booklets for schools, pupils and parent carers
    • the ‘START’ pupil questionnaire - useful for predicting how well children settle in to secondary school.  Secondary schools may wish to use this measure when it is not possible to visit all primary schools prior to the start of the new academic year
    • primary school intervention questionnaires – versions developed for teacher, parent and pupil report to assess interventions used to support all pupils as well as pupils with Special Educational Needs

Thinking Differently for Disadvantaged Learners (TDFDL)

The ‘Thinking Differently for Disadvantaged Learners’ resource is a high-quality online training resource targeted specifically at improving the attainment of our most vulnerable learners across all schools in Worcestershire.

The programme is based around six recorded training modules available to all staff (and governors) to access at their own convenience and/or for schools to incorporate into their whole school professional development plan. It contains over 12 hours of materials presented in bite-sized sessions, designed and produced by leading experts who have a wealth of experience in leading school improvement for disadvantaged youngsters.
