Carer's Support Needs

If you are a Carer and require respite from your Caring role and are looking to arrange replacement care for the person you care for please use the Adult Services Referral button below and your referral will be sent to the relevant social work team to arrange.  If you would also like ongoing support for yourself as a Carer please also complete the referral for the Worcestershire Carers Hub Carer's Support Needs as well as the Adult Services Referral.

Emergency out of hours Social Work Services

The Emergency Duty Team (EDT) provides an out of hours response, on behalf of Worcestershire Social Care Services and Herefordshire Children's Services, to emergency situations that arise outside normal Social Care office hours.

What is Worcestershire Adults Portal?

The Worcestershire Adults Portal is a system used to allow users to submit enquiries and referrals safely and securely to Here2Help and Adult Social Care. The portal's primary role is to receive enquiries and referrals to Adult Services.

How the portal works

The user completes an online form and submits to Here2Help and Adult Social Care. Users will be asked to register for a unique account which allows them to view submitted forms and receive feedback about their referrals.

The login to the Worcestershire Portal and the Worcestershire Children First Referral Portal are the same, you only need to register once.

Please note: Passwords and accounts from the other Worcestershire County Council or Worcestershire Children First services will not work for the portal. Users need to register specifically for this portal or the Worcestershire Children First Social Care Portal.

Benefits include the pre-population of your own details into the referral form and to be able to look back at other recently made referrals which will be kept for 30 days.
