My Worcestershire health plan has 5 big health aims for the next 3 years. The 5 big health aims are to make sure people with a learning disability have a good experience of health services and good health outcomes. The 5 big health aims will have more information about how the plan will work, someone will be in charge of making sure each part of the plan happens
It has been informed and written by people with a learning disability and family carers with Worcestershire’s learning disability health sub group and agreed by the Worcestershire partnership board.
The 5 big health aims are:

Who will be in charge of making sure the plan happens?
The Worcestershire Learning Disability Health sub group will lead on the My Worcestershire health plan, the members of the health sub group include people with a learning disability and family carers. The group must make sure it has the right people to help make the plan work. These people will include the people who make decisions in primary care, community health services, hospital services, and commissioning.
