My Worcestershire health plan

Better health outcomes for people with a learning disability

My Worcestershire health plan has 5 big health aims for the next 3 years. The 5 big health aims are to make sure people with a learning disability have a good experience of health services and good health outcomes. The 5 big health aims will have more information about how the plan will work, someone will be in charge of making sure each part of the plan happens

It has been informed and written by people with a learning disability and family carers with Worcestershire’s learning disability health sub group and agreed by the Worcestershire partnership board.

The 5 big health aims are:

  • big health aim: my GP practice
  • big health aim: my community services
  • big health aim: my hospital visit
  • big health aim: complex needs
  • big health aim: family carers
