is the Reablement
Team?![]()
The Reablement Team is a community rehabilitation team that aims to facilitate hospital discharge, prevent hospital admission, and provide a period of assessment of client’s needs or a period of rehabilitation in their own home.
The Reablement Team’s aim is to help clients achieve their maximal level of independence within an eight-week period. The Reablement Team consists of a full-time occupational therapist, a physiotherapist, and a team of rehabilitation assistants who work seven days a week from 7 am to 10 pm. We also have a full-time customer service adviser who ensures the smooth running of the team.
How does the team work? ![]()
Once a referral is received, one of the therapists will assess the client and establish their suitability for the scheme, referring on to other disciplines as necessary. Goals will be agreed with the client so that involvement is tailored to the clients’ needs and abilities.
For example, a client may initially require four calls a day to assist or supervise with personal care, meal preparation and mobility. On the other hand, they may need help to build up their confidence with everyday activities such as catching a bus to go shopping.
There is close contact between staff and clients, with weekly team meetings and regular therapist reviews. This ensures the efficient and effective progress of the treatment programme, reducing the need for support packages and increasing clients’ practical abilities and general health.
Who
can refer to the Reablement Team? ![]()
Social workers can refer to the Reablement Team, however we will shortly be able to accept referrals from therapists, social workers, GPs and district nurses. We enjoy close links with therapists and encourage discussions with other healthcare professionals involved in a client’s care.
What
are the referral criteria for the Reablement Team? ![]()
Clients must be medically stable, safe to be at home between visits and in agreement to a referral being made. Clients must have potential for improvement, benefiting from occupational therapy and physiotherapy and an intensive rehabilitation programme. They must have the motivation to participate with a rehabilitation programme aiming for functional goals, or the need for a complex multi-disciplinary assessment of their needs.
What happens when
the Reablement Team finishes? ![]()
Before being discharged from the Reablement Team the therapists will discuss with the client, and possibly their carer and other professionals involved, if the proposed goals have been achieved. If there are areas in which the client is continuing to have difficulties the Reablement Team will make appropriate suggestions or referrals. The client will not be discharged until alternatives, if needed, are in place. Each client will receive a questionnaire to comment on the service they have received.
