Worcestershire Young Carers Transitions Service
The Young Carers has now been extended to
include those Young Carers in transition aged between 16 and 24.
The Young Carers service previously offered services to young
carers which ended when they reached their 19th birthday
and they were referred to the adult carers service which many
failed to access as they did not feel it met their needs. It was
recognised that these young adults have specific needs and this
resulted in the transitions pilot service being set up.
The transitions service offers 1-1 support
with education, employment and training being the focus. We
also provide support with a range of needs including emotional
support, help with referrals to other services/agencies and support
to access these services. We also hold a six weekly consultation
group.
Referrals are received from many sources
including the Access Centre, GP’s, colleges, health etc. and we are
also happy to receive self referrals, which can be made to the
Transitions Key Worker.
