Introduction
Worcestershire County Council's Adult and Community Services
Directorate is committed to working with physically impaired adults
aged between 18 and 65 years of age and working with sensory
impaired adults over 18 years of age to promote their independence
and quality of life.
Who do we help?
The Adult and Community Services Directorate has to use its
resources for those in greatest need. We help people with permanent
and substantial physical impairments.
This includes those who have:-
- Terminal Illness
- Severe and profound physical impairments
- Acquired Brain Injury
- Complex and severe health problems
- HIV and AIDS

We provide support to carers, recognising the important
contribution they make towards another's care and well-being, by
offering:-
- Individual assessments to carers
- Practical support and advice

Our services are open to all impaired adults whatever the
gender, race or religion, cultural beliefs or sexual
orientation.
How to get help
You can ask for help yourself or you may ask someone such as a
relative, friend or health professional to contact us on your
behalf. In the first instance contact should be made with the Adult
Duty Team or a Customer Services Advisor at our Access Centre on 0845 607 2000.
We will ask for some basic information about you. This is an
initial assessment, as we need to know more about you to decide if
we can offer you a service.
A Community Care Assessment usually
takes place to identify what your needs are. We will ask you and
your carers to be involved in this as well as other professionals
who may know you.
There is no charge for seeing a social worker but many of the
services we can arrange may require a financial contribution. This
will be explained to you at the time of assessment.
What sort of help can we provide?
The adult physical impairment teams offer a variety of services and
support.

- Residential Care
- Nursing Home Care
- Links with other agencies
- Respite Care
- Meals on Wheels
- Day Services
- Reablement
- Support to access Education or
- Employment Opportunities
- Registration as a Disabled Person
- Blue Badge Car Parking Scheme
- Advice and Information
Direct Payments
Some of these Services may be provided as a Direct Payment allowing more choice and
flexibility.
